Registration Forms

The registration forms are below. We require that you return the following:

1. General student information form
2. Health form
3. Residency affidavit including required personal & property documents

We will also need a copy of your child's birth certificate, social security card, immunization records, and well-child exam.

Dover School

Please email the forms and proof of residency documents to our Office Manager, Lori O'Hern at [email protected], or mail the forms to Dover School, 9 Schoolhouse Rd., E. Dover, VT 05341 prior to August 15th. Your doctor's office can fax the immunization records and well-child exam to the school at 802-464-0562. Wardsboro Elementary School

Please email the forms and proof of residency documents to our Office Manager, Jennifer Finaldi at [email protected], or mail the forms to Wardsboro School, PO Box 107, Wardsboro, VT 05355 prior to August 15th. Your doctor's office can fax the immunization records and well-child exam to the school at 802-896-6687. I look forward to welcoming you into our educational community.
